Specific Skills:
Obtain information and prepare reports or case histories, Assess client's relevant skill strengths and development needs, Conduct follow-up assessments, Develop, coordinate and implement the delivery of specific services within the community, Resolve conflict situations, Liaise with other social services agencies and health care providers involved with clients, Maintain program statistics for purposes of evaluation and research, Assist clients/guests with special needs, Review client background information, interview clients to obtain case history and prepare intake reports. Assist clients to sort out options and develop plans of action while providing necessary support and assistance. Meet with clients to assess their progress, give support and discuss any difficulties or problems. Assists children and youth up to 19 years of age with mental health and addiction challenges to transition from the hospital to the community. Accepts referrals that meet eligibility criteria from the inpatient, outpatient, and urgent care clinic programs. Engages clients and families by quickly establishing therapeutic rapport. Facilitates the initial meeting with the patient and new service provider.

Communicates consistently with other mental health professionals including psychiatrists, psychologist, child and youth workers, social workers, teachers, and nurses. Approaches patient care with a culturally sensitive, stigma-free and trauma informed lens. Establish and maintain professional alliances with collaborating agencies. Providing individualized and/or crisis supports to children, and youth diagnosed with ASD (autism), Developmental Disability, Mental Health Disorder and/or Dual Diagnosis. Engage and build relationships with youth and families by demonstrating interest in the individual, positive reinforcement for behaviours and building self-esteem. Ensure that administrative duties are completed within required timeframes, such as shift and incident reports, and expense and mileage reports.
